Terms & Conditions for Source Promotions as of September 1, 2011.

1. New customers who require equipment from Source Cable Ltd.

Conditions: On approved credit (OAC). When Source Cable provides the equipment, either at no charge (Cable Modems), or monthly rental units (i.e., Digital Cable terminals, High Definition Television terminals), then Source Cable requires proper identification from the customer. Acceptable identification, in order of priority includes: Driver's License, Social Insurance Number and Health Card. Source Cable also requires a $100.00 security deposit on all rented equipment (DCTs and HDTV terminals). A $3.99/MO digital programming fee applies if customer does not subscribe to any digital services.

2. New Super Sports Pak customers

Offer: 1 free month of our Digital Sports Theme package (valued at $9.99/MO plus tax) or any other theme package (valued at $6.99/MO plus tax) if customer already has Sports Package, when customer subscribes to the Super Sports Pak.
Conditions: On approved credit (OAC). Tax not included. Installation fee may apply. This offer cannot be combined with any other offers.There is a 4 month subscription commitment to the Super Sports Pak. Should a customer cancel their Super Sports Pak prior to the 4 month commitment, they will be responsible for whatever monies are owing on the Super Sports Pak commitment (i.e., if they cancel after 2 months, they will be responsible for payment of $29.99/MO for the last two months of the 4 month commitment). Customer requires a Digital Cable Terminal (DCT) in order to receive the Super Sports Pak. DCTs can be purchased or rented from Source Cable. A $100.00 security deposit is required for a rented DCT. Regular price for the Digital Sports Theme package of $9.99/MO will apply at the end of the 1 month promotional period. Expires August 31, 2012.

3. New Home Phone Customers

Home Phone Plus:

Offer: $29.99/MO for 12 months. This represents a savings of between $72.00-$192.00 over 12 months, depending upon what other services the customer subscribes to with Source Cable.
Conditions: On approved credit (OAC). Tax not included. Installation fee may apply. This offer cannot be combined with any other offers.The monthly price of the Home Phone Plus service (after the 12 month promotion period expires) is dependent upon the number of other services (cable television bundle and/or Internet) that the customer subscribes to with Source Cable. Regular price for Home Phone Plus is $45.99/MO when customer does not subscribe to a TV bundle (bundles include TV Pack, Digital TV Pack, Digital Movie Pack, HDTV Pack or the HDTV Movies Pack) or the Internet. It's $40.99/MO when customer subscribes to one of our TV Bundles OR our Internet service (with the exception of Ultra Lite) and $35.99/MO when customer subscribes to one of our TV Bundles AND Internet (with the exception of Ultra Lite Internet). Long distance calls, up to a maximum of 1,000 minutes per month, are for Canada and continental United States only, and do not include calls to Hawaii, Alaska or the Virgin Islands. Home Phone Plus service is only available within our Source Cable service area. Operator Assistance (411) calls are $2.00 per call. International Calling is available and the rates are posted on www.sourcecable.ca. A cancellation fee equal to the discount of between $6.00/MO and $16.00MO will apply to any customer who disconnects prior to the end of the 12 month promotional period (i.e., if the customer cancels after 3 months, the cancellation fee will be for three months savings, from $18.00-$48.00). Offer expires February 29, 2012.

Home Phone Basic:

Offer: $14.99/MO for 12 months which represents a savings of $5.00/MO or $60.00 for 12 months.
Conditions: On approved credit (OAC). Taxes not included. Installation fee may apply. This offer cannot be combined with any other offers. The monthly price of the Home Phone Basic service (after the 12 month promotion period expires) will be $19.99/MO. These monthly rates do not include charges for Long Distance, Collect Calls or 411 fees. Long distance calls in continental North America, outside of our Hamilton local calling area are $0.05 per minute. Call Display is free. Voice Mail, Call Waiting and Call Forwarding are available at $3.00/MO each. Operator Assistance (411) calls are $2.00 per call. International Calling is available and the rates are posted on www.sourcecable.ca. A cancellation fee equal to the discount of $5.00/MO will apply to any customer who disconnects prior to the end of the 12 month promotional period (i.e., if the customer cancels after three months, the cancellation fee will be $15.00). Offer expires February 29, 2012.
The Hamilton local calling area includes: Hamilton, Ancaster, Binbrook, Burlington, Caledonia, Cayuga, Dundas, Dunnville, Fisherville, Freelton, Galt, Grimbsy, Hagersville, Jarvis, Lynden, Mount Hope, Selkirk, Stoney Creek, Waterdown and Winona.

4. New Internet Customers

Offers:
Pro 25: $5.00/MO off the applicable bundled price for 12 months. Save $5.00/MO or $60.00/YR. Expires August 31, 2012.
Extreme 16: $5.00/MO off the applicable bundled price for 12 months. Save $5.00/MO or $60.00/YR. Expires August 31, 2012.
High Speed Internet: $5.00/MO off the lowest bundled price for 12 months. This works out to $35.99/MO for 12 months. Save between $5.00/MO and $13.00/MO ($60.00 to $156.00 over 12 months) depending upon the number of other services (Cable Television Bundle and/or Home Phone Plus) that the customer subscribes to with Source Cable. Expires February 29, 2012.
Conditions: On approved credit (OAC). Tax not included. Installation fee may apply. These offers cannot be combined with any other offers. A cancellation fee equal to the discount of between $5.00/MO and $13.00MO will apply to any customer who disconnects prior to the end of their 12 month promotional period (i.e., if the High Speed Internet customer cancels after 6 months, the cancellation fee will be from $30.00-$78.00). The monthly price of the High Speed Internet services (after the 12 month promotion for High Speed Internet or 12 month promotional period for Pro 25 or Extreme 16, expires) is dependent upon the number of other services (Cable Television Bundle and/or Home Phone Plus) that the customer subscribes to with Source Cable.
Pro 25 is $68.99/MO when customer does not subscribe to a TV bundle (bundles include TV Pack, Digital TV Pack, Digital Movie Pack, HDTV Pack or the HDTV Movies Pack) or Home Phone Plus. It's $63.99/MO when customer subscribes to one of our TV Bundles OR our Home Phone Plus and $60.99/MO when customer subscribes to one of our TV Bundles AND Home Phone Plus. Taxes not included.
Extreme 16 is $58.99/MO when customer does not subscribe to a TV bundle (bundles include TV Pack, Digital TV Pack, Digital Movie Pack, HDTV Pack or the HDTV Movies Pack) or Home Phone Plus. It's $53.99/MO when customer subscribes to one of our TV Bundles OR our Home Phone Plus and $50.99/MO when customer subscribes to one of our TV Bundles AND Home Phone Plus. Taxes not included.
High Speed Internet is $48.99/MO when customer does not subscribe to a TV bundle (bundles include TV Pack, Digital TV Pack, Digital Movie Pack, HDTV Pack or the HDTV Movies Pack) or Home Phone Plus. It's $43.99/MO when customer subscribes to one of our TV Bundles OR our Home Phone Plus and $40.99/MO when customer subscribes to one of our TV Bundles AND Home Phone Plus. Taxes not included.
Lite Internet is $28.99/MO
Ultra Lite Internet is $19.99/MO

5. Triple Play Plus: High Speed Internet, Digital TV Pack & Home Phone Plus

Offer: $120.99/MO for the first 12 months. Save $10.98/MO or $131.76/YR. plus tax. Offer expires August 31, 2012.
Conditions: On approved credit (OAC). Taxes not included. Installation fee may apply. This offer cannot be combined with any other offers. The $120.99 bundled price is in effect for 12 months. Regular price for High Speed Internet is $40.99/MO when bundled with Digital TV Pack and Home Phone Plus. Regular price for Home Phone Plus is $35.99/MO when bundled with Digital TV Pack and Internet. Regular price of Digital TV Pack is $54.99/MO. The conditions outlined in point, point # 3, Home Phone Plus and point # 4 for High Speed Internet also apply to this offer. Customer agrees to keep this Triple Play bundle for 12 months. Should a customer cancel this bundle prior to the 12 month commitment, there will be a $50.00 cancellation fee. Expires August 31, 2012.

6. Quad Play: High Speed Internet, Digital TV Pack, Home Phone Plus and Security

Offer: $133.99/MO for the first 12 months. Save $17.97/MO or $215.64/YR plus tax. Offer expires August 31, 2012.
Conditions: On approved credit (OAC). Taxes not included. Installation fee may apply. This offer cannot be combined with any other offers. The $129.99 month bundled price is in effect for 12 months Regular price for High Speed Internet is $40.99/MO when bundled with Digital TV Pack and Home Phone Plus. Regular Home Phone Plus price is $35.99/MO when bundled with Digital TV Pack and Internet. Regular Price of Digital TV Pack is $54.99/MO. The conditions outlined in point # 3, Home Phone Plus and point # 4 for High Speed Internet also apply to this offer. Source Security customers must commit to monthly monitoring for 36 months when they receive a security system from Source Security. Security customers transferring to Source Security from another alarm company must commit to monthly monitoring for 18 months. Customer agrees to keep this Quad Play bundle for 12 months. Should a customer cancel this bundle prior to the twelve month commitment, there will be a $50.00 cancellation fee. Expires August 31, 2012.

7. Digital Cable Terminal: DCT 700.

Offers:

a). Purchase a DCT 700 for $59.99/tax, a savings of $40.00.
(This price is for the first DCT 700 only. Additional DCT 700s will be $99.99/tax.)
b). Or rent a DCT 700 at $4.99/MO (Source Cable requires a $100 deposit on rented digital cable terminals).
c). Receive 1 free month of three Digital Theme packages valued at $18.00/tax

8. For customers purchasing a Dual Tuner High Definition Television terminal

Offer 1: Purchase a Dual Tuner HD PVR for $399.99 plus taxes, a savings of $100.00.
Customers will also receive 8 months free of our HD 1, HD 2 and HD 3 packages, a savings of up to $136.00.
Total savings including a $100 savings on the HD PVR terminal and HD programming is $236.00.
Conditions: On approved credit (OAC). Tax not included. Installation fee may apply. This offer cannot be combined with any other offers. Offer with programming is for new HDTV customers only. The regular monthly price of the HD1, HD2 and HD3 will begin on the 9th month. Offer expires August 31, 2012.

9. Home builders promotion:

Customers who purchase a home from a local area builder that participates in our home builder program will receive FREE services for a set period of time according to the agreement between customer and home builder, home builder and Source Cable. Some conditions apply. Offer not transferable. This promotion is not applicable for bulk accounts. TV installation charges apply after the fourth outlet. Free telephone installation applies only to existing jacks. Security hardware includes 4 door/window contacts, 1 motion sensor and 2 key pads. Free services must be redeemed at time of install. Any services added after initial installation will be subject to regular fees. Regular terms and charges will apply at the end of the free promotion. 30 days notice is required prior to cancellation of any free services. 12 month security monitoring contract becomes effective at end of promotion.

10. E-Billing

Offer: Swiitch to E-Billing and receive 1 On Demand or PPV movie up to a value of $6.99.
Conditions: The On Demand or PPV movie must be ordered within 30 days of signing up to E-Billing. The $6.99 can be put towards any On Demand or Pay Per View purchase up to a value of $6.99. Expires August 31, 2012.
